Disease Management
Including Diabetes, Arthritis & Parkinson's
If a family member has recently been diagnosed with a debilitating condition like arthritis or Parkinson's disease, it can be a real challenge for you and your loved ones to cope with the new or growing mobility limitations and the capacity to manage basic needs.
We consider it a labor of love to assist our clients with the everyday demands of managing chronic diseases like arthritis, Parkinson's disease, dementia and Alzheimer's disease.
If your family member needs help with everyday homemaking, hygiene, dressing, meal preparation, laundry, or other daily tasks, our caregivers are ready and willing to provide companionship and a helping hand. If your support needs are more extensive, we can provide RNs, nursing aides, physical therapists, and social workers to guide your loved one toward a healthier life.
Our disease management care includes but is not limited to:
- Diabetes medication and blood sugar testing
- Dietary plans to manage diabetes, heart attack, and stroke risk
- Arthritis care and assistance
- Balance rehabilitation
- Personal care and homemaking for patients with Parkinson's
- Dementia and Alzheimer's care
If your loved one requires daily living support as the result of an illness or condition that has been diagnosed by a doctor, you may qualify to receive Medicare aid. Contact us for more information about home care alternatives and Medicare coverage.
