Allegiance Home Health & Rehab Inc. Selected to participate in University of Florida’s Community Health Workers Medication Management Support Project

Boca Raton, FL, May 14, 2014 — Allegiance Home Health & Rehab Inc. will now offer additional services related to Medication Management in order to reduce the risk of Heart Disease and Stroke among at-risk patients while providing support to family members and caregivers.  Already recognized as a HomeCare Elite Organization and as one of the top home health agencies in South Florida, Allegiance is continually striving to improve and  increase the scope and quality of services it provides to the people of Palm Beach, Broward, Martin, St. Lucie and Indian River Counties. Allegiance was selected after a state-wide search by the University of Florida to participate in the Community Health Workers Medication Management Support Project. Minerva Haller-Morella, MSN, RN, Regional Director of Operations completed Part 1 (review of Cultural Competency and Health Care Disparities) of a five part series of Medication Management Training, April 24, 2014 at the UF Research and Academic Center at Lake Nona, Orlando.

The primary objective of the study is to reduce the risk for heart disease and stroke among at-risk patients focusing on the improvement of performance of the ABCS (aspirin, blood pressure, cholesterol, smoking, and salt/sodium) indicators, especially: (i) aspirin for people at high risk, (ii) medication adherence counseling for treatments used for hypertension, hypercholesterolemia, and diabetes, and (iii) smoking cessation counseling or medication use and attention to sodium consumption. Says owner Rosie Inguanzo-Martin, “This is a tremendous opportunity for Allegiance Home Health to partner with an outstanding academic institution implementing support services for patients with high blood pressure and diabetes.
